If you are a new patient, please bring your current glasses, contact lenses, and contact lens boxes with current contact lens information to your appointment.  If you would like to complete your new patient forms prior to your appointment, please print and fill in the New Patient Form Welcome Form and the appropriate Medical History Questionnaire (Age 12 and over or under age 12).  Bring the completed copies to your appointment.

You may order a refill supply of contact lenses using your unexpired contact lens prescription from Yin Eyecare online, call, while you're in our Overland Park office, or email your order below.
